FIRST  VISIT

Welcome to our practice. We are looking forward to meeting you and caring for your periodontal treatment needs. We will be working closely with your general dentist in order to insure the best quality treatment.

    Please help us with your examination by remembering to bring the following:

1.    A completed referral slip, if your dentist gave one to you.

2.   Copies of any recent x-rays that have been taken by your general dentist.

3.   A completed medical history.  A medical history form can be

       downloaded from this website by left clicking here.

4.   Dental insurance information.

 

At this initial examination appointment we will try to give you a general sense as to the nature of your condition and our ideas as to how we can help you. Upon completion of the examination, we will discuss the conditions present and the type of treatment plan best suited to your needs. Our staff will be available to discuss financial arrangements and insurance considerations as well. A comprehensive report of your condition will also be sent to your dentist. Our goal is to provide comprehensive periodontal treatment in a comfortable, safe and caring environment. Remember that periodontal disease is a progressive, painless infection. Delay of treatment can cause further bone loss and increase the cost and time of treatment. Failure to have periodontal disease treated will result in losing your teeth. Dentures are difficult to wear and are never as effective as your own teeth. Fortunately, you are taking the first steps necessary to enjoy the benefits of a healthy dentition.
